Reasonable Accommodation Request Form

The purpose of this form is to initiate the reasonable accommodation interactive process and assist Brown University in determining whether, or to what extent, a reasonable accommodation is necessary for an employee with a disability to perform one or more essential functions of their job safely and effectively. To request accommodations under the Pregnancy Workers Fairness Act, please complete and submit this form. 

A reasonable accommodation is any modification or adjustment to a job or the work environment that will enable an applicant or employee with a disability to participate in the application process or to perform essential job functions. Reasonable accommodation also includes adjustments to assure that an individual with a disability has rights and privileges in employment equal to those of employees without disabilities

This form is to be completed and will reside with the University Human Resources Office of Leaves and Accommodations, and will be handled confidentially, with medical information shared only on a strict need-to-know basis.












Examples;
+Physical functional limitations: difficulty bending, twisting, pushing, pulling, walking, or climbing stairs
+Cognitive functional limitations: difficulty reading, learning number facts, remembering, concentrating
+Psychological functional limitations: difficulty making decisions, problem-solving, understanding oral information

The Americans with Disabilities Act (ADA) defines essential job functions / work activities as the fundamental duties that an employee must be able to perform to hold a position. These functions can be highly specialized, relate to the role's nature, or make up a large portion of the employee's workload.
